Actually the shooting investigation team and his/her supervisor fill out most of the paperwork. The officer writes a statement describing the events that led up to the shooting, and turns that over to the shooting investigation team. The officer also surrenders his/her service weapon or the weapon involved in the shooting, for testing.

Quite a bit, there is an incedent report along with any eye witness reports have been taken and also the general information report, this is quite difficult and lengthy to do, besides it needs to be filled out in triplicate
